Registration Information
Please check back regularly for updated information
Registration fees require a non-refundable $50 deposit per participant. ALL balances must be paid in full by the equipment handout date. If full registration fee has been paid, refunds may be given at refundable levels (see refund policy). Open registration begins on March 1, 2008 and continues until the last registration date scheduled in July or until a team becomes full, after which a child may be placed on a waiting list.
Registration Fee Schedule
Football Players: $175
Tiny Mite & Mighty Mite Cheerleaders: $135
Junior Pee Wee, Pee Wee, Junior Midget, Midget Cheerleaders: $175* (includes manditory $40 Gym Fee)
Each additional child from same family will receive a $20 discount
*Cheerleading squads participate in competitions and additional fees will apply

Completed 2008 Physical Fitness & Medical History Form (must be the Pop Warner form)
Copy of final 2007/2008 school year Report Card

Tiny Mites (TM) : Ages 5,6,7
Mighty Mites (MM) : Ages 7,8,9
Junior Pee Wee (JPW ) : Ages 8,9,10,11**
Pee Wee (PW) : Ages 9,10,11,12**
Junior Midget (JM) : Ages 10,11,12,13**
Midget (M ) : Ages 11,12,13,14,15**
**Older but lighter

Physical Information
Physicals must be completed by a state licensed medical professional. Physicals are required for each participant. Download the Physical Form here. Physicals must be completed using this form. Physicals are available at registration dates for $15.
